Tickets may be purchased on this page (please complete form below for online purchases or pick up registration  at the Welcome Center.  All Attendees must have a completed form.

Notes:

• Tickets will not be issued. Instead, seniors who have paid the fee and turned in their forms will be put on a master list for admission to Grad Bash.

• There are no refunds. Tickets may be transferable.

STUDENT AGREEMENT & LIABILITY RELEASE

Students must arrive and depart from the Grad Bash location by provided transportation. No personal transportation to/from the event will be allowed. No exceptions.

 

I understand that all ticket sales are final. All tickets are nonrefundable. You may transfer to another student. I understand and agree to the policies and provisions of my participation in Grad Bash 2019 of Portage Central High School. I agree not to carry into the party premises any materials or liquids which will affect my behavior and/or will have a detrimental effect on other’s enjoyment of the evening. I understand that I may be patted down for illegal substances. I understand that once I enter the party facility, I may not leave unless my parent or guardian personally arrives at the Grad Bash location to sign me out. (This must be prearranged)

A parent/guardian must sign this liability release regardless of student’s age as this is a PTO event: Liability Release: I understand that my child may be patted down for illegal substances. I agree that the PCHS Grad Bash Committee, PCHS PTO, and/ or Grad Bash Event Coordinator and facility will not be held liable for injury or illness incurred while in attendance at Grad Bash. The undersigned parent/guardian assumes all risks in connection with the student's participation in all PTO sponsored events. In case of injury, and if unable to reach parents by phone, I understand that Grad Bash chaperones and/or employees of the Grad Bash facility will seek medical assistance as deemed necessary.
